ACCIDENT/PARTICIPATION WAIVER I release the event organizers (Team Lyons), City of Copperas Cove, facility owners, and all other parties associated with this event from any liability or responsibility for my physical condition, route of walk/run, for the actions of myself or any other participant. I am aware of the risks of participation and verify that I am physically able to participate in the activity. I recognize various photographs and other media will be taken during the event. I consent and provide the event organizers use of any form for any legitimate purpose to include advertisement. I certify I am over 16 years of age and do not require parental consent to participate.

PAYMENT AGREEMENT

I understand once registration is received ... TeamJlyons will provide me a payment invoice for  registration fees which would be payable upon receipt.
